Winter and Spring Re-Certification:
Any student athlete who plans on participating in multiple sports will need to complete the following re-certification form (Section 5 of the PIAA CIPPE form). Any student-athlete that sustains an injury during the course of a season that requires medical treatment by a doctor (MD or DO) must have Section 6 of the PIAA CIPPE form completed and returned to the Athletic Office in order to return to play. If Section 6 is not completed for an injury, then a re-certification physical exam will be needed to participate in the next sport season.
