Food Service Announcement – Food Allergy Forms & Restrictions on Lunch Accounts

Food Service Announcement regarding Food Allergy Medical Plan of Care Forms and Parent Restrictions on Student Lunch Accounts

By November 1, 2018, all parents should have provided the school a completed 2018-2019 Food Service Medical Plan of Care Form for their student OR updated their student’s prior year Food Service Medical Plan of Care. The Medical Plan of Care must be on file by November 1, 2018 with the building nurse to prevent removal of the student’s food allergy notes from the Cafeteria Point of Sale system. If the food allergy note is removed on November 1st, the cafeteria staff will not have notification of the student’s food allergy. The cafeteria staff will not be able to monitor student’s food selections without notification on the Cafeteria Point of Sale system. Please contact the nurse at your student’s school with any questions.

In addition, on November 1, 2018, all prior year Parent Restriction messages noted on a student’s Cafeteria Point of Sale system will be removed, unless Maria Jones has been contacted in writing via email ( ) or by a note, sent into school to the attention of Maria Jones, Food Services. If you have already contacted Mrs. Jones this year regarding Parent Restrictions on the Point of Sale, please disregard this message.
