- PDE-6004 Arrest/Conviction Report & Certification Form
- Sexual Misconduct/Abuse Disclosure Release (Act 168)
- Credentials/transcripts
- Commonwealth of Pennsylvania Professional Certificate (teaching positions ONLY)
- Three updated letters of recommendation (dated within 1 year)
- Updated application for the corresponding position you are applying for (see below)
- Printable Standard PA Teaching Application (Teaching positions ONLY)
- OVSD Printable Support Staff Employment Application Form (Support positions ONLY)
- OVSD Printable Administrative/Supervisory Employment Application Form (Administrative/Supervisory Positions ONLY)
HOW TO APPLY: Interested candidates must submit a signed cover letter, resume, PA standard application, copy of valid PA certification (if applicable), transcripts, current clearances (dated within one year), and three letters of reference (dated within one year).
